- Savings application form-In a case where the minor cannot sign (below the age of 7yrs) the other party (parent/guardian) to the account becomes the primary account holder and the minor’s information is placed in secondary account holder details.
- Only one (1) Savings Account Form - Minors should be completed and signed by all parties to the account.
- Minors from the age of 7-17 years MUST sign on the savings account form.
- Minors from the age of 7-17 years MUST sign on the savings account form.
- A Personal Information Form to be completed and signed by each signatory to the account.
- An Individual Mandate to be completed and signed by each signatory to the account.
- The Individual Mandate is NOT required for a minor (ONLY persons 18 years & older are required to sign this document).
- A CRS Individual Tax Residency Self Certification Form to be completed and signed by each signatory to the account.
- Minors-Self-Certification–Individual Form MUST be signed by the parent or guardian on behalf of the minor.
- Minors-Self-Certification–Individual Form MUST be signed by the parent or guardian on behalf of the minor.
- No more than four (4) persons can be included on an account
- ALL forms MUST be completed in its entirety.
